Community Support: CCMS (Community Case Management Services) | CREST (Community Resource, Engagement and Support Team) | Henderson Home
- Support the implementation and improvement of activity programmes to ensure relevance and impact.
- Coordinate daily and special programmes with staff, volunteers, and community partners.
- Assist in volunteer management, including scheduling, communication, engagement, and tracking of volunteer attendance and hours using the Volunteer Management System.
- Assist in training, guiding and supervising volunteers during programme activities.
- Be involved in the curation and development of suitable programmes based on residents' needs, interests, and abilities, with guidance and support from the team.
- Prepare programme materials, attendance lists, and activity setup.
- Monitor programme progress, collect feedback, and make improvements for sustainability.
- Record programme outcomes, residents' participation, and programme feedback using internal documentation systems.
Qualifications
You are:
- Currently pursuing an undergraduate degree or diploma.
- A team player with strong communication and stakeholder management skills.
- Detail-oriented, proactive, and eager to learn.
- Proficient in Google Suite applications.
Other Information
Weekend work may be required on an ad-hoc basis depending on needs.
Learning Objectives & Outcomes:
- To be exposed to the planning, coordination, and development of programmes in a nursing home setting, including designing inclusive activities based on residents' needs and abilities.
- To be able to manage programme implementation while ensuring resident engagement and smooth operations.
- To apply communication, leadership, and teamwork skills when working with volunteers and community partners.
- To participate in volunteer management, including coordination, supervision, engagement, and attendance tracking.
- To gain knowledge in programme evaluation, documentation, and sustainability planning.
