Our Story

As a charity healthcare institution with the primary mission of providing affordable medical, nursing and rehabilitative care services for the seniors in the community, our Strum of care has expanded over the years to support the growing healthcare needs from the silver generation.  

• Establish rapport and build professional relationship with patient and significant others (e.g. family, caregivers)
• Conduct needs and intake assessment to identify the severity and key barriers and protective factors for interventions.
• Engage patients, families, multidisciplinary team and relevant stakeholders to gather holistic and relevant information.
• Analyse and evaluate different courses of action in terms of their immediate and long-term impact.
• Formulate appropriate strategies for action
• Set clear goals with patients and engage patients to sustain effort in problem solving.
• Collaborate with patients, families, multi-disciplinary team and relevant stakeholders, including participation in case conference, to develop and review care and rehabilitation plans throughout the care duration.
• Collaborate and negotiate with stakeholders to mobilise community resources.
• Provide documentation and timely updates of psychosocial assessments and interventions in medical record.
•  Conduct closure of cases upon achieving sustainable service outcomes for patients, families and communities.
