Primary Care Coordinator – Job Description 

Coordinates and Schedules Diabetic Foot Screening Services, Diabetic Retinal Photography

Competent in performing Diabetic Retinal Photography Services(training will be provided )

Provides education, prepares, pre-empts caregivers, family members on disease process, disease management and develop or tailor individual health plans and to change health plans according to needs

Tracks, Monitors and follows up on the quality of care provided by GPs and other Primary care professionals and to ensure maintenance of good clinical care.

Competent in data entry in Chronic Disease Registry and provide onsite training to Clinic assistants

Conduct Quarterly audits to ensure data is entered correctly

Assists GPs in Submission of data and other relevant administrative work

Involved in PCN planning team to ensure smooth coordination within PCN

Conducts PCN briefing and training to newly enrolled clinics and provides onsite assistance
