JOB DESCRIPTION

Personal and Psychosocial Care

• Provides support to seniors in basic activities of daily living (BADL), including assistance with personal hygiene and transfer.
• Identify care needs of seniors such as dietic needs and other areas relating to well-being of elders.

Nursing Support

• Attend to basic nursing and general care needs of clients
• Monitor health status (vital signs, glucose level and oxygen saturation) and escalate to Centre Supervisors/ Nurse Manager.
• Perform tube feeding / Nasogastric tube (NGT) feeding for seniors
• Perform basic wound care
• Assist to serve medication/ pre-packed medication and apply prescribed topical medication as instructed by Supervisor/ Manager.

Rehab Care

• Assist Physiotherapist/ Occupational therapist in clients prescribed therapy.
• Assist to manage movement dysfunction and enhance physical and functional abilities.
• Assist to restore, maintain, and promote optimal physical function, optimal wellness and fitness movement.
• Assist in client ambulation movement within the centre and during transferring to and from transport vehicle.

Senior Engagement

• Plan, and conduct activities suitable for seniors taking into consideration of their preferences and interests
• Observe senior’ engagement during sessions and feedback on senior’s response and progress.
• Work with the therapists on implementation of new activities
• Provide support and involve in the planning and coordination of Active Ageing Programmes

Administration and Operations

• Always ensure environmental safety and hygiene of the centre including carrying out the necessary cleaning and disinfection and looking out for hazards
• Able to do basic documentation of senior’s participation in various engagement activities.

REQUIREMENTS

Experiences:

• Experience in ILTC sector is an advantage
• Therapist assistant cert would be an added advantage

Qualifications:

• GCE ‘N’ Level and above
