Job Description
- Establish rapport and build professional relationship with patient and significant others (e.g. family, caregivers)
- Conduct needs and intake assessment to identify the severity and key barriers and protective factors for interventions.
- Engage patients, families, multidisciplinary team and relevant stakeholders to gather holistic and relevant information.
- Analyse and evaluate different courses of action in terms of their immediate and long-term impact.
- Formulate appropriate strategies for action
- Set clear goals with patients and engage patients to sustain effort in problem solving.
- Collaborate with patients, families, multi-disciplinary team and relevant stakeholders, including participation in case conference, to develop and review care and rehabilitation plans throughout the care duration.
- Collaborate and negotiate with stakeholders to mobilise community resources.
- Provide documentation and timely updates of psychosocial assessments and interventions in medical record.
- Conduct closure of cases upon achieving sustainable service outcomes for patients, families and communities.
